Assumption of risk & release of liability

In consideration of being permitted to participate in a Mobile Gaming Suite event operated by Flour Mound Consulting LLC ("Company"), I acknowledge that participation is voluntary and involves ordinary risks. On behalf of myself and any minor children I am signing for, I agree to the following:

I assume the ordinary risks of participating in and being around the gaming trailer and equipment.
To the fullest extent permitted by law, I release and hold harmless the Company, its owners, employees, and contractors from claims for property damage or personal injury arising from the event, except to the extent caused by the Company's gross negligence or willful misconduct.
I agree to supervise my children, to use the equipment as directed by the coach, and to be responsible for damage caused by my or my guests' misuse or negligence, as described in the Terms of Service.
I confirm the information below is accurate and that I am the participant or the parent/legal guardian authorized to sign for the minors listed.
